Childbirth Basics The cervix doesn't begin to dilate until the start of labor. The first stage of labor is usually the longest, lasting 6 to 18 hours for first pregnancies and 3 to 10 hours for subsequent births. It begins when your cervix starts to open and ends when it's fully dilated. The second stage - which can last two hours or longer - commences when your cervix is fully dilated and continues until your baby is born. The last stage takes about 15 to 20 minutes and ends when the placenta is expelled.
